This was a template by The Thadman which I felt could use a few minor changes and wanted to use for my own simple wikification. The original can be seen at User:The Thadman/Template/Wikification. All credit goes to The Thadman.
How it works
As with The Thadman's template, usage is as follows:

Which gives:

Then for each stage that the article fulfils the criteria for, add a |pass, a |fail for what it doesn't meet, and anything else will appear in brackets like so:
